Patient Safety in Medication Dispensing

At Medvantx, patient safety is our top priority. As we dispense millions of patient prescriptions daily across our two pharmacy locations, we take extensive measures to maintain the highest quality standards. Our dedication to patient safety is reflected in every step of our pharmaceutical dispensing and medication fulfillment processes.

Temperature-Controlled Shipments

One of the key aspects of our commitment to patient safety is ensuring that medication orders are shipped with temperature tags. These tags prove that the order was maintained at appropriate temperatures throughout the shipment. This is crucial for preserving the integrity and efficacy of the medications. Additionally, all pack outs for all programs are qualified pack outs that have proven they are validated to ship over the designated shipping periods.

Cold-Chain Shipments

Our production workflow is meticulously designed around patient safety in medication dispensing for our cold-chain shipments to reduce the overall time out of refrigeration. We use tested and approved cold chain shippers to ensure that safe medication is delivered to patients. This process is vital for medications that require specific temperature conditions to remain effective.

Standard Operating Procedures (PSOPs)

We follow strict PSOPs to ensure that medications are picked, packed, and manifested/shipped following appropriate procedures. Our production is barcode-scan driven to ensure accuracy. This technology helps us verify that the right medication is dispensed to the right patient at the right dose, route, and time.

The Five R’s of Dispensing Patient Medications

  1. Right Patient: Ensuring that the medication is dispensed to the correct patient is crucial. This involves verifying patient information through multiple checks, such as confirming the patient’s name, date of birth, and medical history. This step helps prevent medication errors and ensures that the patient receives the treatment intended for them.
  1. Right Medication: This step involves verifying that the correct medication is being dispensed. Pharmacists check the medication name, strength, and dosage form against the prescription to ensure accuracy. This helps prevent dispensing errors and ensures that the patient receives the appropriate medication for their condition.
  1. Right Dose: Ensuring the correct dosage is critical for patient safety. Pharmacists verify the prescribed dose against the patient’s medical history and current medications to avoid potential drug interactions or adverse effects. This step helps ensure that the patient receives the optimal therapeutic dose.
  1. Right Route: The route of administration (e.g., oral, intravenous, topical) is verified to ensure that the medication is delivered in the most effective manner. Pharmacists check the prescription instructions and educate patients on the proper administration techniques to ensure the medication’s efficacy and safety.
  1. Right Time: Timing is essential for medication effectiveness. Pharmacists ensure that the medication is dispensed and administered at the correct time, as prescribed. This includes verifying the dosing schedule and providing patients with clear instructions on when to take their medication.

By adhering to these Five R’s, Medvantx ensures that every step of the medication dispensing process is meticulously checked and verified, significantly reducing the risk of errors and enhancing patient safety.

These principles guide our pharmacists in reviewing every order at two separate, unique times. The first check is to review all clinical information, and the second is to review product accuracy and integrity.

Quality Checks and Monitoring

Quality checks and monitoring are performed daily in our Kentucky and South Dakota locations for picking, packing, and manifesting. These checks ensure that every step of our process meets the highest standards of patient safety. “We also support patient safety event reporting to our pharmaceutical manufacturer partners and strive continuously to improve by learning from events and creating action plans,” -Amanda Chuk, Director of Quality

Patient Welcome Packet

As part of our ACHC accreditation for quality assurance, we have developed a Patient Welcome Packet. This packet helps patients have a clear pathway to our pharmacists for questions, medication concerns, and information on how to get started with Medvantx PAP programs. It’s designed to make the patient experience smoother and more informed, ensuring they have all the necessary resources at their fingertips.
